This model also provides individuals with strategies to effectively cope, regulate, and transform their emotions and feelings, while building tighter bonds with the people that matter most. My name is Kimberley Durr, and I am a Psychiatric Mental Health Nurse Practitioner. I graduated from Rush University College of Nursing in Chicago, IL and am board certified. I evaluate and treat mental health disorders across the lifespan with medication and have experience working with children, adolescents, adults, and the elderly. I am most experienced in mood disorders, anxiety, ADHD, and schizophrenia but have experience in other neurodevelopmental, eating, substance use disorders. In my practice I approach each person individually and holistically. I incorporate his/her life's narrative and preferences when developing treatment plans and provide psychoeducation regarding my diagnostic impression. I offer a compassionate, gentle approach to patient care and provide guidance that is evidence based. I became a nurse simply because I like helping those in need. It brings me great joy and satisfaction to know I am making a difference in someone's life. I eventually found a position working in mental health and never looked back. I was a psychiatric nurse for a few years before pursuing my role as a nurse practitioner. This field can be challenging but rewarding, and I am determined to improve the lives of others. This first session involves getting to know each other and gathering history and collateral information when needed. At the Heart
At the Heart is a community counseling center whose goal is to bridge the gap that exists in access to services and to help heal our community, families, and individuals. All members of our community need respect and acceptance. Our counselors provide a caring and non-judgemental environment for clients to get the support and encouragement they need, when they need it. We provide both traditional/secular counseling and Christian based counseling, determined by a client's preference. We meet our clients where they are at and work collaboratively to help each client achieve their individual goals. We have several different kinds of providers. We have a Peer or Lay Counseling Division which is an every day person that is passionate and has a heart for helping others, life experience, and has completed a 13 week (40 hour) counseling course through At the Heart. The course addresses depression, trauma, anxiety, suicidality, grief/loss, child abuse, domestic violence, addiction, and mindfulness. These providers could be nurses, teachers, stay at home parents, retirees, and so on. They have an open heart and a willingness to listen and encourage. We have a Professional Division which involves Trainees, Associates/Interns, and Pastors. Trainees are Graduate/Doctoral school students in the field of counseling that need to acquire hours towards graduation and licensure. Associate and Interns are Post-Masters/Doctoral Graduates that are accruing hours towards licensure in Marriage and Family Therapy, Social Work, or Professional Clinical Counseling. All of our counseling staff receive weekly case consultation and supervision by a California Licensed Marriage and Family Therapist that has special training in supervising non-licensed clinicians. San Jose Behavioral Health Outpatient Services
San Jose Behavioral Health offers Intensive Outpatient Program (IOP) and Partial Hospitalization Program (PHP) for adults 18+ with a primary mental health diagnosis. Our PHP services are Monday-Friday from 9am-3pm, while we offer IOP services either 3 or 5 days per week, from 9am-12:30pm. While in treatment, individuals will engage in group therapy with the addition of individual sessions weekly with a therapist, psychiatrist and registered nurse. Treatment is an average of 6-8 weeks long, with individuals who begin in PHP stepping down to IOP prior to graduation. Currently we are offering services a both in person and via tele.
